List of documents confirming fulfillment of the conditions for admission to limited tender

1. Valid excerpt from the relevant register or valid certificate of entry in the business activity register, or – if the Contractor is based outside the Republic of Poland – documents issued in its country of registration and confirming that the Contractor is not in liquidation or bankruptcy; the excerpt, certificate and documents must be issued not earlier than six months before the deadline for applying for admission to the tender procedure.

2. Current certificates from the Tax Office, or – where the Contractor is based outside the Republic of Poland – documents issued in its country of registration and confirming that the Contractor is not in default with payment of taxes; the certificates and documents must be issued not earlier than three months before the deadline for applying for admission to the tender procedure.

3. Current certificate from the Social Insurance Institution, or – where the Contractor is based outside the Republic of Poland – documents issued in its country of registration, confirming that the Contractor is not in default with payment of social security and healthcare contributions and charges; the certificate and documents must be issued not earlier than three months before the deadline for applying for admission to the tender procedure.

4. Insurance policy or another document confirming that the Contractor has business liablility insurance.

5. Balance sheet and income statement for the last two financial years.

6. Current information from the National Penal Register, or – where the Contractor is based outside the Republic of Poland – a certificate from the relevant court or administrative body of the Contractor’s country of registration, containing information specified in Art. 24.1.4-8 of the Public Procurement Act; the information and the certificate must be issued not earlier than three months before the deadline for applying for admission to the tender procedure.

7. List of executed projects whose scope corresponds to that of the proposed contract, along with documents confirming proper performance of these projects. The list shall be accompanied by credentials.

8. Where the request for admitting to the tender procedure is submitted by a consortium, a copy of the consortium agreement.

9. EU VAT tax identification number (for foreign Contractors).

10. Representation to the effect that the Contractor has the technology required for execution of the proposed contract.

11. General or specific power of attorney granted to natural persons, if they are not entered in the companies register as persons authorised to represent the company, or to legal persons (in the case of a consortium).

The documents should be in the Polish language and they should be delivered in the original or authenticated as a true copy of the original by the Legal Counsel or persons authorised to represent the bidder. If the person’s country of origin or the country where the Contractor is based or domiciled does not issue the documents referred to in items 1, 2, 3 and 6, instead of these documents the Contractor shall submit documents containing representations made before a notary public, competent court, administrative body or professional or commercial self-government body of the country where the Contractor is based or domiciled
